Community Services Care Coordinator – Overview

Engage community members and families with empathy following a violence incident. Identify client needs and provide referrals; coordinate services with other providers. Assist clinicians in service linkage, coaching, and skill‑building. Maintain effective communications with stakeholders. Establish rapport and work cooperatively with children, families, CII, and community resources. Ensure accurate and timely completion of outcomes and reports. Maintain knowledge of service standards, policies, contracts, licensing, and governing bodies. Advocate on behalf of clients with agencies and government programs. Participate in initial intake interviews and complete required paperwork. Set up and maintain files and documentation, ensuring compliance with policies. Duties – Additional
Conduct home visits to determine client needs; provide training, guidance, and support. Participate in multidisciplinary teams to develop and implement service plans.
