SOCIAL WORKER (MSW) - CASE MANAGEMENT

Beebe Healthcare . Beebe Healthcare is seeking a Social Worker (MSW) for case management to provide professional social work services as part of the interdisciplinary healthcare team. Responsibilities
Performs psychosocial assessments with patient and family that are consistent with age and developmental status (per department standard). Works with others to ensure a valid patient Advanced Directive is completed and documented. Engages with uncooperative or challenging patients/family members to gain cooperation regarding treatment and discharge goals. Reviews information about patient\'s proposed living arrangements and evaluates security, hazards, and resources to assess discharge readiness. Prepares patients for discharge or movement between living arrangements. Performs age-specific discharge planning designed to meet aftercare needs, involving patients/families as much as possible in the planning. Informs the physician, nurse, and other health care team members about the discharge plan as appropriate. Educates patients and families regarding community resources to maintain health status and address ongoing psychological issues. Develops appropriate social work intervention plans based on psychosocial assessment findings. Coordinates medical, social, or financial support services to ensure an efficient care process. Contacts agencies by letters, memos, e-mail, or phone to refer patients/families to services. Communicates with health care team members about issues affecting care processes or outcomes. Communicates patient progress and ongoing care needs to post-discharge caregivers. Assists patients/caregivers with follow-up appointments and steps if medical attention is needed before then. Uses resources effectively to ensure timely discharge to the appropriate level of care. Other duties as assigned. Qualifications
Master of Social Work (MSW) is required. 2 years of clinical social work healthcare experience, preferably in a med-surg hospital setting. Experience with PC, mainframe, or Internet-based information systems. Knowledge and experience with Electronic Health Records; Cerner and Curaspan preferred. Complete Case Management Certification within 3 years of hire. Essential Competencies
Clear written and verbal communication skills. Ability to maintain confidentiality of patient information. Ability to handle crisis situations. Customer service orientation for patients, team members, and visitors. Knowledge and experience with Electronic Health Records. Education

USD $63,648.00/Yr. to USD $98,654.40/Yr. Employment Details
