Pay Rate:
$20.00 - $21.66
Schedule:
Monday – Friday: 8:30 a.m. – 4:30 p.m.

About Us With a history stretching back more than 70 years, AccessCNY is continually growing and expanding our reach. Offering person-centered services that empower individuals of all ages and abilities to reach their full potential as part of our community. Our focus is supporting individuals by ways of residential housing, mental health and clinical services, and immersing our participants in community engagement. Working closely with the Office for People with Developmental Disabilities and the Office of Mental Health, we support 27 housing locations and 3 administrative buildings.
About the Role We are looking for a proactive employee to assist participants enrolled in Community Habilitation services to achieve independence, as well as maintain their health and safety.
Responsibilities
Implement participants’ goals as written in life/staff action plans
Assist/support participants with ADL skills as needed, including, but not limited to: bathing, toileting, dressing, dining, mobility, etc.
Teach participants skills to promote independence, including, but not limited to: cooking, money management, literacy, socialization, health, safety, etc.
Offer choices and opportunities for participants to engage in during their free time and during community activities; support community inclusion
Provide opportunities for participants to increase decision-making and advocacy skills
Transport and accompany participants to meetings and activities of their choice
Monitor and maintain participant safety in the participant’s home, community, and during transportation
Follow procedures to report emergencies and/or medical needs/changes of participants
Documentation
Complete program documentation in a timely manner per regulation and Agency policy; documentation includes, but is not limited to: service notes, behavioral data, program outcomes, etc.
Complete incident report forms thoroughly and in a timely manner per Agency policy
Maintain purchase receipts and complete necessary documentation per Agency policy
Complete mileage sheets per Agency policy
Facilitate positive communication with other departments and accept assignments in other areas as requested by supervisor
Work collaboratively with participants, families, program staff, co-workers, and supervisor
Follows Agency Standards and Procedures
Perform Other Duties as Assigned
Education/Experience No education or experience necessary. High school diploma/GED, preferred. Experience working with people with disabilities, preferred. Must be able to demonstrate the ability to appropriately communicate with participants via ASL, as well as document services through process chosen by the Agency. Basic word processing and email skills required. Fluency in American Sign Language is required.
Conditions of Employment Must have a valid New York State driver’s license in accordance with the Agency Vehicle and Driving policy and transportation necessary to meet program needs. Must be able to lift 75 pounds.
