Pay Range Base pay: $48,000.00/yr - $48,000.00/yr
Location & Remote Work This is a full‑time remote position with travel required between Suffolk and Nassau County. The home office will be West Babylon.
Role Overview Utilize the Individual Placement and Support (IPS) Model for individuals enrolled in ACT and CORE services. Responsible for vocational and career assessment, benefits counseling, job development and placement, and job retention support for individuals with mental health diagnoses. Spend a minimum of 50% of work hours off‑site in the community.
Responsibilities
Conduct vocational and career assessments for clients.
Provide benefits counseling and support.
Develop and execute job development activities.
Facilitate job placement and monitor job retention.
Travel between Suffolk and Nassau County as required.
Participate in community outreach and networking.
Qualifications
High School Diploma or equivalent (Bachelor’s Degree preferred).
Minimum 6 months of professional or personal experience in human services.
Fingerprinting, criminal record check, approval from NYS Office of Mental Health, and a Clean NYS Driver’s License.
