**Target Salary Range:** $51,000-$63,800**Position Summary:**The Senior Education Specialist is responsible for providing education case management services to Choice Neighborhoods (CN) families with children aged to five and grades K-12 to support academic achievement and enhance behavioral outcomes as outlined in the Choice Neighborhoods Education Roadmaps.

Education case management services include outreach to families and students; eligibility determination; assessments; individual development planning; referrals to supportive services providers to enhance student success in school; service coordination; ongoing monitoring, coaching, and counseling; retention; and tracking achievement of outcomes.

The primary target population for case management services will be former residents of the target development site, new residents of the redeveloped site and other AH-assisted residents in the Choice Neighborhoods community.

The ideal candidate will demonstrate strong alignment with Atlanta Housing mission and vision, embodying our core organizational values through:* Exceptional analytical skills and sound professional judgment* A proactive and strategic mindset with anticipatory problem-solving abilities* Deep commitment to organizational goals, ethical standards, and youth development* High levels of personal and professional integrity* Energy, adaptability, and resilience in a dynamic environment* A collaborative spirit and commitment to inclusive engagement* Proven ability to execute impactful initiatives and deliver measurable results* A passion for empowering youth and transforming lives**Essential Duties and Responsibilities:*** Deliver case management and wraparound services that include conducting office and home visits; assessing clients’ social services needs using tools provided by AH; assisting clients with student development plans that will encourage improvement in attendance, behavior and/or academic deficiencies; referring students to supportive services providers and other organizations offering services that meet client needs; monitoring client participation in programs and services and progress in achieving goals and outcomes; using AH’s designated database system to consistently and accurately record client demographics, case management activities, referral & participation status, and progress in achieving goals and outcomes; coaching and counseling clients to support retention and goal achievement.* Develop necessary relationships with the CN families and coordinate efforts to serve students through collaboration with the primary AH Case Manager and the Atlanta Public School (APS) Liaison to identify and connect CN families with academic and community resources.* Manage a caseload of up to 100 households annually achieving the overall goal for the delivery of case management services as detailed in the grant application.* Ensure delivery of case management services are streamlined with focus on achieving the overall goal of the Choice Neighborhoods Education Strategy of delivering services and supports to improve student’s overall academic performance with a special focus on improving educational outcomes and intergenerational mobility for youth with services and supports delivered directly to youth and their families.* Educate clients on how to access community-based supportive services on an ongoing basis.* Work collaboratively with Data Analyst to validate case management data and reporting to ensure accuracy.* Achieve, maintains and communicates expert knowledge on community-based service provider resources, the services they offer, and eligibility requirements in order to ensure accuracy and effectiveness in referring clients to services that meet their needs.* Effectively achieve Key Performance Indicator (KPI) metrics as defined by Manager.* Perform other duties as assigned.**Education and Experience:*** Bachelor’s degree in social services or a related field, required.* Four (4) years of case management experience to include experience working with low-income individuals and/or families.* Preference given to individuals with experience as a School Social Worker, School Counselor or Behavioral Intervention Specialist**Knowledge, Skills, and Abilities:*** Demonstrated ability to manage up to 100 households caseloads with varying issues and challenges.* Demonstrated ability to utilize case management technology to maintain accurate, comprehensive, and up-to-date information on client caseload activity.* Must exhibit good judgment, tact, and diplomacy when dealing with internal and external customers.* Strong verbal and written communication skills. Ability to interact with colleagues at all levels in a professional manner delivering complete and accurate information.* Demonstrated ability to plan, organize, and prioritize efficiently in an environment of multiple and shifting priorities.* Strong interpersonal skills and ability to work collaboratively with other team members.* Ability to work independently and fully execute assignments without intensive supervision.**Working Conditions**

Works in both an office and field environment. May engage in frequent community-based meetings for client outreach, including weekends and evenings. May require some bending, stooping, reaching and prolonged sitting. May require some lifting usually not exceeding 10 lbs. Must have and maintain a valid state of Georgia driver’s license and be fully insurable for the purpose of obtaining clearance on company “Authorized Drivers List”. #J-18808-Ljbffr
