Responsibilities
Available to cover shifts as assigned at any residential program, including rotating to other programs as needed.
Facilitate ADL skill-building for residential living and community integration; supervise and teach clients daily medication routines, budgeting, self-preservation skills, transportation, etc.; provide opportunities for skills practice and collaborative learning.
Participate on a multi-disciplinary team in the development and implementation of client treatment plans and progress reviews; engage clients in planning and evaluation.
Provide, coordinate and monitor treatment services for all assigned clients as prescribed in the treatment plan; document contact and activities with clients, families, and other staff and collaborators.
Monitor the effectiveness of care by comparing services provided with outcomes as specified in the treatment plan.
Provide shift coverage including household maintenance, cleaning and meal preparation; assist in crisis resolution, conflict resolution and symptom management.
Attend supervision and team meetings; maintain knowledge of agency policies and procedures.
Available to work first and second shifts, holidays and weekends; ensure safety and security of office areas and residential programs.
Schedule and facilitate treatment plan meetings; use assessment information and records to develop and document assessments and treatment plans/reviews in accordance with policy.
Screen for substance abuse and provide assessment with indicated treatment planning, counseling/education and referrals.
Perform other duties as assigned; remain on shift until appropriately relieved (may involve consecutive shifts as designated).
Minimum Qualifications
High school diploma or GED required.
Maintain flexibility toward work schedules; ability to work posted hours, weekends and holidays.
Ability to perform routine household chores involving some lifting and bending.
Ability to communicate with clients and staff face to face and by telephone.
Ability to obtain and maintain CPR certification and first aid training; monitor client’s vital signs as required.
Maintain a valid driver’s license and ability to transport clients as needed.
Job Knowledge, Skill, and Ability
Ability to read and comprehend instructions, correspondence and memos; ability to complete clinical and administrative documentation.
Ability to write correspondence, reports, care plans, etc.; present information effectively in one-on-one and group settings.
Ability to access emergency resources to manage crisis situations and ensure safety of clients and staff.
Maintain current knowledge of organizational policies, procedures, protocols and guidelines to ensure compliance.
Ability to solve practical problems with limited standardization; interpret various instructions in written, oral and diagram form.
Oral and written communication skills sufficient for effective communication; ability to perform arithmetic calculations as needed.
Physical/Environmental Demands The role requires typical physical activities including use of hands, walking, sitting, reaching, climbing, standing; may involve lifting up to 35 pounds; work may occur in outside conditions with moderate noise. Reasonable accommodations will be provided under applicable law.
Pay Range $17.00 - $26.60
