Position Summary:
The Care Navigator, under the Strengthening Behavioral Health Outcomes (SBHO) grant, provides intensive case management and care coordination services to individuals with mental illness or substance use disorders, focusing on housing stability, benefits assistance, and access to community resources. This role assesses client needs, removes barriers to engagement, and collaborates with community providers to promote recovery, independence, and successful reintegration.
Key Responsibilities & Role Highlights:
The Navigator will be placed within the Cherry Street Ministry Mission.
The position works to assist clients (assigned caseload) to secure housing within the community and assists in monitoring their placement to ensure stability of housing.
Performs case management duties, assisting clients with overcoming barriers of obtaining and maintaining resources. Maintains confidentiality and ensures release of information is handled in accordance with federal guidelines and agency policy/procedures.
Participates in team meetings, supervision, in-service training, and conferences according to established policies, procedures, and applicable regulations.
Education & Experience Requirements:
High school diploma required. Four‑year degree in social work, psychology, or related field preferred.
Minimum of a CDCA license required. LCDC II/III or LICDC preferred.
Must be able to work a flexible schedule that may occasionally include evenings and weekends.
