Job Description Responsible for providing direct services as a clinician, assisting with supervision of specialty roles based on evidence‑based practice, trauma‑informed care, cultural humility, and person‑centered language, and providing administrative oversight to the team in conjunction with the Team Leader. The Assistant Team Leader conducts individual and group supervision, collaborates with a multidisciplinary team, and supports all facets of client care and program operations.
Essential Accountabilities
Co‑leads daily team organizational and service planning meetings, provides direction to staff, and conducts individual and group supervision.
Meets required productivity targets and completes all documentation within assigned time frames.
Monitors and coaches staff on productivity measures.
Directs and coordinates the client admission process and treatment, rehabilitation, and support services.
Participates in staff recruitment, interviewing, hiring, work assignments, orientation, and performance supervision.
Develops and implements staff orientation and training.
Develops and maintains program policies and procedures and revises as necessary.
Performs on‑call duty for crisis intervention and staff supervision.
Oversees accurate use of internal electronic health record (Avatar).
Provides guidance in engagement and communication with enrollees and identifies barriers to care.
Audits client charts on a regular basis.
Coordinates services with AOT, mental health, and criminal courts, probation, and child protection services.
Documents progress notes in EHR within 48 hours of all client and collateral contacts.
Communicates pertinent information to the Deputy Director, Division Director, and regulatory bodies.
Conducts field‑based visits and attends case conferences, case consultations, and ACT orientations/training.
Supplies and monitors data entry in VelloHealth and ensures staff maintain up‑to‑date records.
Provides crisis intervention and mental health counseling as needed.
Assists in planning and implementing goals and training for participants, including but not limited to budgeting, grooming, nutritional planning, transportation, community resources, shopping, housekeeping, physical health & development, adaptive behavior/independent living, nutrition, vocational development, social development, and after‑care recommendations.
Other Responsibilities
Maintains required written documentation and communicates relevant information verbally and in writing.
Works flexible schedules; trainings and travel may be required.
Operates an agency vehicle as needed.
Attends all required training and activities deemed necessary by supervisors.
Participates in staff activities during shortages and maintains confidentiality at all times.
Reports and documents all incidents to immediate supervisor within 24 hours.
Qualifications
Master’s degree or higher in social work, psychology, rehabilitation counseling, or a related field with licensure/registration (LMSW, LCSW, LMHC, RN, NP, MD, PsyD, etc.). Preference for multidisciplinary mobile team experience.
Two years supervisory experience with all levels of staff and experience meeting OMH and other governmental regulatory requirements.
Computer proficiency, including email and all documentation in an electronic medical record.
Strong interpersonal and communication skills; knowledge of group dynamics required.
Valid New York State driver’s license and vehicle; regulatory background checks required.
