Position Rehabilitation Specialist – Full Time

Location Springfield, MA

Rate of Pay $26.00 per hour

Hours 40 hours per week

Schedule Monday – Friday 8:00am – 4:00pm with flexibility

Signing Bonus

$500 sign on bonus ($250 at the completion of first week of training and $250 after successfully completion of first 90 days)**

Summary Responsible for gathering data, documenting and presenting each member’s goals and objectives in accordance with ISP regulations; coordinating services with case managers and parent/guardians of program member’s goals and objectives, issues, concerns and abilities; and ensuring the provision of high quality services. The Rehabilitation Specialist is also responsible for overseeing the screening and prioritizing of referrals; documenting member progress; implementing behavior management strategies; investigating incidents and all other duties as assigned.

Qualifications

High School diploma with 5 years related experience OR Associate’s degree with three years related experience OR Bachelor’s degree with one year related experience.

Knowledge of and respect for diverse populations.

Previous experience working in human services preferred.

Requirements

2 years of previous case management experience required.

Ability to use computers to include Microsoft Word, data entry, email and internet essential.

Experience in Credible preferred.

Must be organized, detail-oriented, have solid time management skills and be able to work in a fast‑paced multi‑tasking environment.

Demonstrated ability to deliver high level of customer service/satisfaction with both internal and external customers.

Excellent communication, interpersonal and listening skills.

Must have a valid driver's license, good driving record, use of vehicle and willingness to transport participants in personal and/or agency vehicle.
