The Rehabilitation Specialist plays a vital role in helping our participants build resilience by completing rehabilitation assessments, actively participating in treatment planning, and implementing rehab services.
What a Rehabilitation Specialist Does At Resilient Health
Gathers community resources and provides encouragement for clients to participate in activities promoting community integration.
Educates team members on the value of meaningful community activities, including employment, psychiatric rehabilitation, work incentives, and benefit planning.
Facilitates participants' connection, referral, and access to employment and rehabilitation services.
Assesses, coordinates, monitors, and provides direct supportive and skills based services in a community setting. This includes utilization of the Vocational Activity Profile or similar tool to assist participants identify interests and connect them to the necessary supportive services.
Contributes in the development of the participant's individual service plan (ISP) and coordinates services related to the ISP goals.
Utilizes the following resources and tools: Disability Benefits 101, Employment Status within the Supplemental Member Data Provider (DUG) Portal, RSA Vocational Rehabilitation program, Interagency Service Agreement (ISA)/ Collaborative Protocols, AHCCCS Covered Behavioral Health Services Guide Rehabilitation related including limitations.
Requirements For This Position
Behavioral Health Technician (BHT) required (must be 21 years old):
High School Diploma or equivalent and two years of full-time behavioral health work experience, or
Associates Degree and six months of full-time behavioral health experience, or
Bachelor's Degree, or
Master's Degree
Valid Arizona or other state driver's license.
Valid automobile insurance.
Valid CPR/First Aid certifications or ability to obtain within 30 days of hire.
Valid Fingerprint Clearance Card or ability to obtain within 90 days of hire.
Three‑year Motor Vehicle Report that meets our auto insurance carrier's coverage requirements.
Bilingual preferred but not required.
