Overview

Under the supervision of a licensed nurse, the Certified Nursing Assistant (CNA) provides daily personal care, assists with meals, housekeeping, and recreational activities, and supports residents' psychosocial and hygiene needs. In the Dogwood Memory Care neighborhood, the CNA works collaboratively with the household team to support dining, programming, and the upkeep of residents' living spaces, ensuring a safe, clean, and engaging environment.

Essential Functions

Provides personal hygiene (bath, shower, shampoo, assistance with elimination needs, dressing, or other personal care to include but not limited to shaving and fingernail clipping and filing) and takes vital signs as directed by the plan of care; reports any observations or change of condition to charge nurse. Follows residents' plan of care and respect residents' choices. Reviews the resident's plan of care with the charge nurse with any new assignment, changes or when additional explanation or teaching is needed. Communicates all changes in resident care needs with Charge Nurse. Creates and maintains an atmosphere of warmth, person-centered care, personal interest and positive emphasis, as well as a calm environment throughout. Provides physical comfort and a safe environment. Follows all safety and security policies and procedures; performs all tasks to ensure resident and personal safety and the protection of co-workers and facility property. Performs safe transfers and assists with ambulation safety using assistive devices. Accurately weighs residents as assigned. Promptly and courteously answer residents' requests for assistance and responds to "call for aid" as directed. Assists with dining room duties, including serving the residents their meals, monitoring the dining room, delivering trays and assisting with filling out menus and submitting them to dining services. Prepares residents for appointments and assists with transportation within the community; accompany to outside appointments as needed. Always utilizes standard precautions and follows transmission-based precautions when appropriate. Reports any potential exposure or hazardous conditions to the charge nurse promptly. Completes all annual education requirements. Completes all required electronic and paper documentation timely with accuracy. In the Memory Care neighborhood, assists Life Enrichment department with programming that mirrors normal life activities of residents (to include basic household chores and the upkeep and management of personal living space) mental and emotionally engage their interest and assist them in recapturing the value of normal community life.

Assists residents in keeping their living areas and personal belongings clean, organized, and in good working conditions. Supports, leads, and participates in a resident-centered life enrichment activity program. Perform other duties as assigned.

Education and Experience

High school diploma or equivalent is required. Valid State of Connecticut Certified Nurse's Aide certification is required. Must be able to speak, read, and write the English language in an understandable manner. Experience with MatrixCare or other electronic medical records is preferred. Environment and Working Conditions

Ability to focus on task needs and perform intellectual executive functions as related to documentation, charting, care device operations, etc. Ability to execute intellectual tasks and duties under conditions of high demand and distraction. Ability to complete the essential functions of the position with or with adaptive personal devices (e.g., hearing aid, corrective lenses). Ability to operate equipment and technology required for the position. Able to wear personal protective equipment. Must be able to communicate effectively. Able to assist with the evacuation of residents under emergency status to the best of their physical ability. Able to function independently with integrity and work effectively with residents, personnel, and support agencies. Able to constantly stand or walk to provide resident care and assistance. Able to frequently push and/or pull a force up to 30 pounds including residents weighing up to 250 pounds in wheelchairs and. Able to reach overhead, bend, and carry or lift 50 pounds occasionally and 35 pounds frequently. Able to frequently transfer and support residents weighing up to 250 pounds while positioning, and transferring residents to beds, toilets wheelchairs, etc. Additional employees or mechanical assistance may be used. Able to frequently reposition residents weighing up to 250 pounds in bed for assistance with treatments, hygiene, and comfort needs. Additional employees or mechanical assistance may be used. Hours

Per Diem:

1st: 7:00am - 3:30pm

2nd: 3:00pm - 11:30pm

3rd: 11:00pm - 7:30am

Pay Rate

$20.00 - $24.00 per hour (based on years of experience) + shift differential ($1.50 p/h weekday evenings, weekday nights & weekend days, $2.00 p/h weekend evenings & weekend nights)
