Case Manager - QMHP Integral Care – Austin, TX

Job Summary Case Managers provide psychosocial rehabilitative skills training and case management to individuals diagnosed with severe mental illnesses who may also have co‑occurring substance use disorders and/or chronic health conditions. Using a recovery‑based and trauma‑informed perspective, QMHP staff operate in a dynamic environment, develop extensive knowledge of community resources, collaborate with interdisciplinary teams, and cultivate a clinical skill‑set by forging therapeutic relationships with diverse community members. This position is site‑based at a permanent supportive housing (PSH) program for single adults with a history of chronic homelessness and a disabling condition. Use of a company vehicle and/or a personal vehicle is required.

Responsibilities

Work with racially and culturally diverse individuals to define and facilitate their desired life outcomes.

Participate in diagnostic evaluations and provide assessment of strengths, preferences, functional impairments and symptoms of psychiatric and/or co‑occurring disorder(s) at regular intervals to ensure optimal and personalized services.

Coordinate services with external and internal providers to support individuals in achieving independent functioning.

Deliver therapeutic interventions that foster skills necessary to manage symptoms, obtain/maintain employment or housing, or obtain services such as school, medical care, nutritional assistance, financial assistance, transportation, legal assistance and other basic needs.

Facilitate psychosocial rehabilitative skills training groups and/or utilize other evidence‑based interventions or therapeutic modalities.

Provide services under the supervision of an LPHA who provides ongoing evaluation and oversight of all assessments and treatment.

Provide on‑call support as required by unit protocols and offer bilingual communication services when required, passing a language competency test if applicable.

Maintain regular, predictable attendance and demonstrate flexibility to work a flexible schedule if required.

Qualifications Education

Graduated from an accredited college or university with a Bachelor’s degree in Human/Behavioral/Social Services or a related field. Master’s degree preferred.

License

Current and valid driver’s license with a clean driving record.

Experience

Bilingual preferred.

Work Schedule

Full‑time.

Monday – Friday, 8:00 AM – 4:30 PM.

Salary: $24.18 – $25.39 an hour based on education.
