* Manage multidisciplinary teams including certified peer counselors who have lived experience in behavioral health recovery, outreach and in-reach workers, housing specialists and mental health professionals.* Provide staff with training and resources to build relationships with eligible people in the community and help connect them with supports including housing, transportation, and health care.* Promote outreach and case management services for people who have significant barriers to accessing behavioral health and mainstream community supports.* Monitor the provision of housing support including housing subsidies; transportation assistance; support for accessing health services; and independent living skills training as applicable.* Coach and guide staff to assist Trueblood class members with accessing independent living options, including those who would benefit from short-term housing assistance beyond the 14‐day vouchers.* Ensure staff provide timely assistance to clients assessed to need housing support beyond what is offered through the crisis triage and stabilization facilities or the use of short-term voucher.* Develop community partnerships with law enforcement, local shelters, faith-based services, community court, jail and other entities that could assist in case finding efforts.* Work collaboratively with SPD and SFD, House of Charity, Eastern State Hospital, Providence Sacred Heart Medical Center and other emergency departments, and other community partners involved in serving Trueblood class members.* Continually seek areas of opportunity for program and process improvement and work in collaboration with the director to implement positive change.* Provide and role model effective leadership that engages, motivates and supports assigned staff.* Provide weekly supervision to assigned staff in a manner that promotes and develops a positive environment for members and staff.* Oversee implementation and adherence to organization-wide and department specific FBH policies, monitoring for compliance and addressing non-compliance as needed.* Assist staff to utilize productive conflict resolution techniques in handling conflict.* Monitor staff performance to ensure staff meet performance expectations; productivity requirements, compliance with required training, documentation and time keeping. Address employee performance concerns in a timely manner, utilizing HR support as needed. Complete employee performance reviews in a timely manner.* Escalate barriers, high risk situations, unresolved concerns, etc. to Director or appropriate individual as needed.* Oversee and ensure adherence with contracts related to assigned program. Maintain and provide required data as specified in the contract.* Provide direct clinical services and cover shifts as required.* Participate in peer review, supervision, intake and diagnostic staffing, in-house educational and administrative meetings as directed.* Be involved in quality improvement projects to assist the team in being successful at diverting inpatient admissions.* Supervise and train staff in achieving a level of expertise required to perform telephone triage, crisis intervention, motivational interviewing, trauma informed care, and treatment duties and monitor and evaluate clinical proficiency thereafter.* Requires travel to see clients in the community and travel between work sites either via FBH owned Vehicles and/or personal vehicle.* Other duties as assigned.* Demonstrated ability to train and supervise staff in clinical service delivery, case conceptualization.* Knowledge of models of crisis intervention, community-based intervention for children and/or adults with serious and persistent mental illness.* Must have effective communication and interpersonal skills.* This position provides client services in-office and within the community at large on an frequent basis.* Community settings include but are not limited to residences, group/congregate living settings, hospitals, shelters, the streets or anywhere in the community at large.* Some environments may not be ideal in terms of cleanliness or being habitable.* Frequent travel between locations is required.* The ability to frequently drive a vehicle, traverse distances up to several city blocks and on various terrains, navigate stairs, get in and out of vehicles y is required.* Daily use of a computer is required.* May be required to lift 10 - 20 pounds.* Blood-borne Pathogen Category III - No predictable exposure to blood-borne pathogens.* Medical, Dental and Vision* Retirement – We offer an impressive 403b plan including standard 3% employer contribution plus an additional 3% employer match.* Flexible Spending Accounts* Life Insurance – Company paid coverage that starts the first of the month following 6 months from your date of hire.
