Patient Engagement Specialist

We are seeking a Patient Engagement Specialist to drive new patient acquisition and growth. The role works closely with partnering cardiology practices to screen eligible patients, educate them about the Karoo Care Program, enroll them, and coordinate care transitions to our virtual team.
Responsibilities
Maintain daily presence at partner cardiology practices and support additional clinics remotely as needed.
Pre‑screen patients for eligibility and notify practice providers of potential enrollments.
Verify patient eligibility, elicit provider feedback, and conduct enrollment onboarding sessions.
Document activities in the Karoo Health Kohere.AI system and partner cardiologist systems.
Provide patient education and a program overview to ensure understanding of benefits.
Coordinate with practice staff for admissions, readmissions, and ED visits data.
Manage inbound patient calls regarding enrollment and ongoing engagement.
Implement enrollment strategies and assist in transferring patients to the virtual team.
Present program information to practice partners and support ongoing stakeholder communication.
Execute special projects and other assigned duties as needed.
Qualifications
High school graduate or equivalent.
Community Health Worker, Medical Assistant, LPN, or similar experience.
Cardiac‑related experience preferred.
Minimum three years of related experience in health care or physician clinic/practice settings.
Understanding of enrollment and billing processes.
Strong interpersonal skills and ability to quickly establish rapport with providers and patients.
Comfortable working within teams and adapting to fast‑paced environments.
Exceptional written and verbal communication skills.
Ability to use data to inform decisions.
