TITLE OF POSITION: CERTIFIED ACTIVITY COORDINATOR
CATEGORY: NON EXEMPT
RESPONSIBLE TO: EXECUTIVE DIRECTOR
LIFTING REQUIREMENTS: 50 LBS.
GENERAL DESCRIPTION:
A person assigned the responsibility of planning, organizing and executing treatment with orientation in activities. Also responsible for such activities with all participants in the program. Experience or background in Arts and Crafts activities is a plus.
RESPONSIBILITIES:
Plan, organize, execute and/or direct therapeutic activities including Arts and Crafts, using craft media, music, games, and exercise to the greatest benefit of each participant. Assess and evaluate participant’s progress and record participation in progress notes. Plan and post activity calendar according to DAHS requirements. Perform other related duties as required. Initial and periodic assessments, communication of participant changes to IDT team members, development of plan of care. Meet documentation requirements in the medical record and the timeframe for the completion of each assessment and plan of care.
MINIMUM QUALIFICATIONS:
Educational Activity Director Certificate with functional knowledge of craft media; Arts and Crafts experience strongly preferred. Previous clinical experience with population similar to participants preferred. Verbal ability to instruct participants, and to implement an appropriate therapy program requiring familiarity with medical terminology. Sincere interest in working with people and developing a program to have a therapeutic benefit for each participant. Ability to objectively assess participant’s progress and communicate with treatment team. Ability to work cooperatively with all professionals to provide maximum services to participants. Ability to handle a variety of duties which may be interrupted and changed by immediate circumstances.
Two years of experience in a social or recreational program providing and coordinating services for a frail or elderly population.
