$45,000 to $47,000 per year / Full Time

Responsibilities
Maintain a thorough, current knowledge of all state and federal rules, laws, and regulations pertaining to operation of an ICF/IID.
As appropriate, monitor the management of medications and medical supplies by the employees.
As appropriate, monitor the management of appointments with outside providers.
Accurately complete Individual Assessment forms quarterly and maintain necessary supporting documentation.
Conduct annual and special Individual Plan meetings for each resident on their caseload.
Complete and update floor book information to reflect the current status of each individual.
Complete all Individual Plans, Comprehensive Functional Assessments, Quarterly Reviews and Special Team Meeting minutes for individuals served.
Coordinate and ensure utilization of community services for individuals based on their preferences and needs.
Requirements
Four-year degree in social services or related field.
One year of experience in the field of developmental disabilities may substitute IDD education for experience.
Strong customer service and communication skills.
Supervisory or management experience.
Displays strong communication skills and possess excellent decision-making and time-management skills.
