Key Responsibilities
Advocate for resident rights and serve as the facility's Ombudsman
Support residents and families through adjustment, illness, disability, and end‑of‑life care
Act as the facility’s Grievance Official and coordinate customer satisfaction efforts
Assess and document psychosocial needs and coordinate community resources
Assist with advance directives, care planning, and discharge planning
Participate in MDS, care planning, QAPI, and interdisciplinary team meetings
Serve as the facility’s Abuse Officer and ensure regulatory compliance
Maintain accurate documentation and support survey readiness
Collaborate closely with facility leadership and staff
Qualifications
Licensed Social Worker preferred (state licensure if required)
If not a licensed social worker, a Bachelor’s degree is required
BSW or a closely related field is highly preferred
Experience working with individuals and families in crisis
Knowledge of geriatric care, medical settings, and community resources
Familiarity with Medicare, Medicaid, and private insurance resources
Strong communication, organizational, and interpersonal skills
Ability to handle emotionally demanding situations with professionalism and compassion
Ability to meet all health, background, and regulatory requirements
