Role Description The Outreach Executive (OE) is responsible for developing relationships with members of the community to engage Medicare seniors and help these individuals become patients of Oak Street Health. OEs work closely with Outreach Managers, Outreach Directors, and Community Relationship Managers to drive new patient growth in their assigned territory.

Core Responsibilities

Sales

Generate leads by engaging Medicare seniors through event execution and local community outreach.

Maintain knowledge of internal sales systems and marketing programs; attend scheduled meetings for updates on services, workstreams, and initiatives.

Prospect and cold call prospects and new leads assigned to you. New leads may come from digital channels or other Outreach Executives.

Nurture prospective patients via email, phone, and networking with an ambitious mentality.

Collaborate with your Community Relationship Manager (CRM), other OEs, and Outreach Manager/Director to drive lead generation with local community members or Insurance Agents.

Relationship Management

Maintain consistent touchpoints with prospects to continuously engage them and drive new patient growth in the assigned territory.

Capture and record customer information and sales activity into the customer relationship management (CRM) system and other business systems.

Act as point of contact for clinical or center staff if they have questions about a patient.

Resolve concerns or complaints from prospects or patients by working with them to understand the issue or directing them to another Oak Street employee.

Perform other duties as assigned.

What We’re Looking For

Ability to quickly connect and influence the right people.

Ability to manage priorities simultaneously.

Team player and contributor to the overall effectiveness of the team.

Sales and/or healthcare experience is a plus.

Fluency in Spanish, Polish, Russian, or other languages spoken by people in the communities we serve (where necessary).

US work authorization.

Time Type Full time

Pay Range $18.50 - $31.72 per hour. The base hourly rate or full‑time salary depends on experience, education, geography, and other factors. The position is eligible for a CVS Health bonus, commission, or short‑term incentive program.
