At Haven Home Health & Hospice , we believe exceptional care starts with strong leadership. We’re looking for a Clinical Director to guide our home health team and ensure that every patient receives the highest quality of care, delivered with compassion, clarity, and commitment.
As Clinical Director , you’ll oversee patient care services, support our skilled clinical team, and collaborate closely with each department to maintain excellence in care and compliance. This role offers the opportunity to make a meaningful impact on both patient outcomes and staff growth in a supportive, locally owned environment.
What You’ll Do
- Oversee all patient care services and clinical staff
- Coordinate patient care, assignments, and referrals
- Ensure individualized plans of care are developed and updated
- Provide clinical oversight and leadership to maintain quality and efficiency
- Support agency performance improvement and clinical outcomes initiatives
- Participate in team meetings, supervisory visits, and care conferences
- Manage daily huddles and clinical operations in collaboration with the Executive Director
What We’re Looking For
- Current RN license (required)
- Minimum of one year of recent clinical or management experience (within the last three years)
- Strong clinical judgment, communication, and leadership skills
- Excellent organizational and time management skills
- Proficiency with electronic documentation systems (OASIS experience preferred)
- Home health experience strongly preferred
