
Help Me Grow Home Visitor
EAST CENTRAL OHIO SPECIAL EDUCATION RESOURCE CENTER, Frankfort, Kentucky, United States
The East Central Ohio ESC is seeking a compassionate individual to work with culturally diverse populations within Tuscarawas County as a Help Me Grow Home Visitor. This position will be responsible for initiating and maintaining regular and long-term (up to five years) contact/support with the families. Interventions will be family-centered, strength-based, and focused on:
Establishing a trusting relationship
Assisting in strengthening the parent-child relationship
Assisting parents in improving their skills to optimize the home environment
Improving the family support system
Increasing the family’s ability to problem-solve and assume the role of advocate for themselves and their children
The Home Visitor will also be responsible for delivering an evidence-based parenting education curriculum and assisting the family in establishing goals and a plan for accomplishment of those goals, as well as ongoing screenings and assessments of the normal growth/development of the target child.
Minimum Qualifications
Associate’s degree in a field of student relating to child and family studies, child life, early childhood education, social services, child development, nursing or other applicable healthcare fields required
Bachelor’s degree in social work, education with a focus on early childhood, or nursing preferred
LSW, professional teaching license or RN license preferred, but not required
Knowledge of infant/child development and experience using child development assessment tools
Working knowledge of community resources
Open to reflective practice (i.e., has capacity for introspection, communicates awareness of self in relation to others, and recognizes value of supervision
Valid State of Ohio driver’s license and proof of insurance
Acceptance of individual differences
Experience and willingness to work with the culturally diverse populations within Tuscarawas County
Responsibilities Duties include, but are not limited to the following:
Determines family’s eligibility to receive Help Me Grow Home Visiting Services
Assures that every family has a completed family-directed plan with at least one family goal as it relates to any of the four goals of the Help Me Grow Home Visiting program within mandated timeliness
Informs parents of their rights
Verifies that developmental screenings of the child(ren) have been completed. If a delay is suspected, completes a referral for a developmental evaluation with the consent of the parent
Completes required tools used for parent/home assessment
Facilitates and participates in the development, implementation, review and monitoring of the Family Plan and their timelines
Facilitates development of family outcomes
Uses the Growing Great Kids curriculum for parent education
Assists parents in identifying, establishing and communicating with a medical/health home
Identifies and connects families to specific need-based resources and providers
Informs families of the availability of advocacy services
Coordinates with medical and health providers
Coordinates transition of child(ren) to development-enhancing program/early care and education provider by age three
Responsible for maintaining appropriate documentation for the client record (hard copy and electronic data systems) according to ODH and local rules/policies
Communicates and cooperates effectively, professionally, and respectfully with families/schools/professionals, and all others involved with the Help Me Grow system
Contributes to team effort by integrating individual responsibility with other team member responsibilities
Demonstrates commitment to continuous learning and self-improvements by reading and attending such meetings, conventions, and conferences as are necessary to keep abreast of the latest Help Me Grow developments and trends

260 Day Contract, Full-time
Starting Salary Range – $30,000-$35,000, but will be based on education & experience
