Job DescriptionJob DescriptionAbout the RoleWe are an established, independently owned Medicare-certified home health agency in San Antonio that has been open since 1996 seeking a hands-on Director of Nursing to lead clinical operations, ensure compliance, and support continued growth.This is not a corporate DON role. Our DON is actively involved in day-to-day clinical oversight, quality assurance, staff support, and survey readiness. We are looking for someone who values structure, accountability, and patient safety and understands the realities of home health.Key ResponsibilitiesProvide overall clinical leadership and oversight for nursing servicesOwn and manage the QAPI program and quality improvement initiativesEnsure compliance with Medicare Conditions of Participation, Texas HHSC rules, and agency policiesOversee clinical documentation and support OASIS accuracy (coding outsourced; DON-level understanding required)Supervise, mentor, and support RNs and LVNsParticipate in survey readiness and respond to audits as neededCoordinate with intake, scheduling, and administrative staffBe available for occasional field visits (spot checks, coverage when needed)Support growth while maintaining compliance and clinical qualityRequired Qualifications (Non-Negotiable)Active Texas RN license in good standingMedicare Home Health experience (minimum 2–3 years)Prior leadership experience (DON, ADON, Clinical Manager, QA Nurse, or Supervisor)Strong understanding of:Medicare home health regulationsClinical documentation standardsQA/QAPI processesComfortable working in a small-agency, hands-on environmentStrong organizational and communication skillsPreferred (Strongly Desired)OASIS SOC/ROC/Discharge experienceSurvey or audit experiencePsych or complex patient experienceAxxess, Kinnser, or similar home health EMR experienceBilingual (English/Spanish)What This Role Is — and Is NotThis role IS:Hands-onCompliance-focusedCollaborative with ownershipDesigned for someone who wants autonomy and real impactThis role is NOT:A corporate or regional DON positionA desk-only roleA hospital transition role with no home health backgroundCompensationSalary range: $85,000 – $100,000 (based on experience)Paid Time Off (PTO) offeredMileage reimbursement for work-related travelPhone stipendNo corporate bureaucracy; direct access to leadershipWhy Join UsStable, established agency with strong compliance historyOpportunity to shape clinical operations and growthRespect for clinical judgment and leadershipSupportive, experienced administrative teamFocus on doing things the right way, not cutting cornersHow to ApplyQualified candidates with Medicare home health leadership experience are encouraged to apply.
