Social Worker Director

to join our team. This is an exciting opportunity to join our mission driven organization to provide excellent care to our residents. This role will lead our long term care and rehab social work team. Responsibilities

Director of Social Work (Long Term Care & Rehab) responsibility includes leadership and organization of the Social Work department; provide administrative and clinical supervision to social workers and discharge planners to ensure quality of service to the long term care and rehabilitation units for Stella Maris. Provide direct social work services to nursing home and rehabilitation patients and families as needed. Ensure departmental documentation is in compliance with State and federal regulations for the department. Plan, develop, organize, implement, evaluate and direct the operations of social service programs for the facility. Promote the mission, vision, and values of Stella Maris. Develop, plan and implement program goals, objectives, personnel, resources and equipment. Develop and implement policies and procedures for identifying the medically-related social and emotional needs of the patients/residents. Assist in providing solutions to social and practical environmental problems, including seeking financial assistance, discharge planning (with collaboration with community agencies) and referrals to other community agencies when specialized assistance is required. Provide supervision to staff that meets regulatory compliance. Provide clinical consultation, oversight of case planning and care coordination. Manage a caseload and direct crisis intervention efforts as required. Provide staff management including hiring, development, training, performance management and communication to ensure effective and efficient department operation. Ensure procedures and workflow are consistent with organizational objectives, treatment protocols, regulations and standards of care. Ensure regulatory compliance with MDS, BIMS, PHQ9, and Care planning. Requirements

Licensed Social Worker in the State of Maryland required Degree in Social Work required Prior experience in long term care strongly preferred Prior supervisor experience preferred Benefits
