CareCore at Lima is seeking a compassionate and experienced Social Services Director to lead our social services department. Our 88-bed skilled nursing facility is committed to providing the highest quality of care and support to our residents and their families. We’re looking for someone who is passionate about making a difference in the lives of others and thrives in a collaborative team environment.
Responsibilities
Develop and implement social service programs that meet the emotional and social needs of residents
Provide support and counseling to residents and families
Coordinate discharge planning and community resources
Maintain documentation and records in accordance with state and federal guidelines
Serve as an advocate for residents and ensure their rights are protected
Work closely with interdisciplinary team members to promote resident well-being
Participate in care plan meetings and contribute to care planning
Qualifications
Current, active Social Work license in the state of Ohio Preffered but not required
Minimum of 2 years experience in a long-term care or healthcare setting
Strong leadership, communication, and organizational skills
Knowledge of applicable state and federal regulations
