Position Overview As an LSW, you will help oversee care planning and ensure residents’ psychosocial needs are met with compassion and professionalism. You’ll act as a vital bridge between residents, families, facility staff, and community resources.
Responsibilities
Assess and address the social and emotional needs of residents
Develop and manage individualized care and discharge plans
Serve as a liaison among residents, families, medical staff, and external agencies
Maintain accurate documentation, including MDS 3.0 and CAA’s
Advocate for residents' rights and support ethical care practices
Coordinate psychological services and hospice referrals as needed
Participate in interdisciplinary care planning and quality improvement efforts
Ensure compliance with state and federal regulations regarding social services
What We’re Looking For
Licensed Social Worker (LSW) - Preferred
Experience in a skilled nursing or long-term care setting preferred
Strong communication, organizational, and leadership skills
Knowledge of community resources and elder care best practices
A passion for working with seniors and making a meaningful impact
