About the Role The CoNEXTions Street Outreach Program supports youth ages 12-21 who are experiencing, or at risk of, homelessness, abuse, trafficking, or exploitation. Outreach Specialists meet youth where they are—in the community, on the street, or in our drop-in center—offering safety, resources, and ongoing support. The work is grounded in trauma-informed care and emphasizes trust, collaboration, and empowerment.
What Youall Do
Build positive, professional relationships with youth in the community Conduct street outreach and identify youth who may need support Provide basic-needs items, information, and a safe point of contact Help youth with life skills, housing, education, employment support, and other services Complete assessments, documentation, case plans, and progress notes Support drop-in center services and group activities Provide transportation for clients as needed (agency or personal vehicle) Collaborate with internal programs and community partners Participate in required training and meetings Identify individuals who may benefit from other agency services or community services and make appropriate referrals to assist individuals in accessing those services Minimum Requirements High School Diploma/GED/HSE At least one year of experience with at-risk youth or individuals facing homelessness preferred Valid driver?s license (MO Class E or IL Class D), auto insurance, and acceptable driving record Reliable personal transportation About Community Counseling Center We\'ve served our region since 1974, providing comprehensive behavioral health services across five counties.
