
Resident Service Director - LPN (Education)

As the Resident Services Director, you’ll lead our health and wellness initiatives ensuring each resident receives compassionate, personalized, and high-quality care. Your Role in the Legacy Mission As the Resident Services Director, you are the heartbeat of our health and wellness program. You’ll oversee care planning, resident assessments, and daily clinical operations, ensuring our community remains a place where safety, comfort, and compassion thrive. Through your leadership, our care team will uphold Legacy’s promise to honor every story and nurture every soul.
What You’ll Do
Meet with residents regularly to provide health and wellness checks and address any health-related questions.
Complete assessments prior to move-in, upon condition changes, and as required by policy or regulation.
Review medical reports before move-in to ensure the residence can meet individual care needs and regulatory standards.
Oversee medication management, including ordering, scheduling, and ensuring accurate documentation.
Develop and maintain individualized service plans for each resident.
Maintain ongoing communication with residents, families, physicians, and outside healthcare providers.
Coordinate additional health services and support transfers to or from hospitals or other care facilities.
Counsel residents and families with empathy and professionalism.
Participate in the on-call rotation for medical questions and resident emergencies.
Collaborate with the Food Service Director on residents' dietary needs.
Review health changes with the Executive Director and physicians to ensure continuity of care.
Ensure infection control protocols and wellness standards are consistently met.
Maintain accurate and confidential wellness records for all residents.

Who You Are
A compassionate, organized, and detail-oriented healthcare professional committed to excellence in senior care.
Licensed nurse (LPN or RN) preferred.
Previous experience in assisted living, senior care, or healthcare management required.
Skilled in assessment, care planning, and medication management.
Strong leadership and communication abilities.
Able to manage multiple priorities with empathy and efficiency.
Must pass background checks and meet all applicable state requirements.
