Requirements
Intake Coordinator Duties:
• Coordinates enrollment into PIHC'S Emotional Wellness & Recovery Department (EWR) for patient's seeking individual counseling,
group counseling, psychiatry, peer services, and recovery services.
• Determine eligibility of patients seeking EWR services at PIHC and complete behavioral health consultations.
• Enters patient information and documentation into the EMR.
• Schedules behavioral health intake appointments for patients with intake clinicians utilizing the EMR system.
• Checks Ryan White eligibility by collecting eligibility documentation.
Patient Retention Specialist Duties:
• Abides by all state, health district and agency policies regarding confidentiality of patient information
• Monitors health outcomes of Rapid Entry Clinic patients and assists with data collection and reporting.
• Monitors show rates for all appointments including follow-up with the long-term care provider to verify that the initial medical visit occurred
• Collaborates and consults with PIHC clinic staff, medical case managers, and Behavioral Health staff for care coordination.
• Attends all interdisciplinary staff meetings
• Collaborates with community partners and resources in the Metro Atlanta area.
• Conducts follow-up calls with patients.
• Checks Ryan White eligibility by collecting eligibility documentation.
• Develop a sustainable HIV care plan with each client that focuses on removing barriers to care, determining benefits, and linking
to a long-term care provider
• Facilitates linkage to a long-term care provider by reviewing provider options with the client and making appropriate referrals
• Identifies patients who are out of care and/or not enrolled in EWR within the past 6 months.
• Conduct reminder phone call/text between pt consult & intake appt
• Have Interim check in phone calls for pts who have fallen out of care
• Conduct follow up contacts 24hrs after a missed intake appointment
• Conduct exit interviews for discharged pts
• Conduct meetings for pts who are not consistently engaged in services & assess barriers to care
• Work with pts care team to identify pts who have fallen out of EWR care and develop strategies to get them back into care
• Retention activities that include but not limited to: calls, letters, meetings, etc.
• Assist with arranging transportation for pts who are eligible
• Community partnerships to coordinate linkage, retention, re-engagement (hospitals, detox, jail, etc.)
Minimum Qualifications:
• Master's Degree in Counseling, Social Work, Psychology, or related mental health field required.
• Knowledge of Electronic Medical Record (EMR) system
Experience:
• One-year experience in social services or mental health agency and/or equivalent combination of training and experience which
provides the required knowledge, skills, and abilities.
• Completion of 700 hours of supervised practicum/internship in counseling/psychotherapy
• Professional experience working with individuals living with HIV preferred
License/Licensure:
Licensure eligibility required (LPC, LAPC, LCSW, LMSW, Licensed Psychologist, LMFT); fully licensed preferred
