ARISE/Exceptional Family Resources Independent Living Specialist

Independent Living Specialist, Olmstead ARISE/Exceptional Family Resources is looking for a Independent Living Specialist to provide support to individuals who wish to move from nursing home residence back to living in the community. The program provides support services and rental assistance to eligible individuals in Onondaga and surrounding counties. The Independent Living Specialist identifies goals and creates a plan with each program participants to coordinate community-based services and provide meaningful one-on-one support. This position works closely with the program's Housing Specialists to maintain stable housing in a person's chosen community setting. Required Qualifications: Associates degree required, bachelor's degree preferred, or equivalent combination of education and related experience Excellent communication and organizational skills Experience working with individuals with disabilities required Solid knowledge of available community-based services Proficiency in Word and Excel, general computer literacy to navigate databases, use email and other required electronic systems Valid NYS Driver's License Workdays and Times: M-F 8a-4p Location: Syracuse Hourly rate: $20.16-$21.16/hour Checkr Background Check
