Position Summary

Independent Optometrist – Sam’s Club (Independent Optometrist Leasing Space). Launch your services in Sam’s Clubs and take a leading role in patient care with respect to eye and vision care as well as general health and well‑being.

As an independent lease holder you will have full autonomy to:

• Establish your days of practice
• Set your hours of operation
• Control your patient schedule
• Determine your fee schedule
• Keep 100 % of exam/professional fees

• Option to receive administrative and/or opt‑tech support services (where state allows)
• Lease agreements managed by Gumberg Asset Management Company for ME & VA licenses

Qualifications

• Doctor of Optometry (OD) from an accredited optometry school
• Valid state optometry license
• MD or DO from an accredited ophthalmology school
• Valid state ophthalmology license
• General Liability Certificate
• Professional Liability Insurance Certificate
• W‑9 form

Preferred Qualifications

Optional: any additional experience or certifications that enhance patient care and practice management.
