Your Role As an Outreach Specialist, you are responsible for locating and connecting with patients who would benefit from Synapticure’s care model, and engaging with them around their needs. These conversations are a patient’s first encounter with Synapticure, after which they are connected to a Care Coordinator to develop a long‑term relationship. However, until the transition to their Care Coordinator is made, you are the patient’s point person, available and eager to assist with whatever they need: support, education, or a listening ear. You work alongside, and are supported by, Synapticure’s care team to ensure patients and caregivers have the support they deserve, right from the start.

Essential Functions

Engaging eligible patients through telephonic, written, and digital outreach methods

Leveraging available data from EMR to learn more about potential patients and inform outreach processes

Completing the intake and consent process for eligible patients, and transitioning them to a Care Coordinator upon full enrollment

Engaging and fostering trust quickly with patients, acting as the face of Synapticure and the key point‑of‑contact to assist patients prior to full enrollment

Connecting authentically with patients and their families in a non‑judgmental, respectful, and empathic manner and striving to meet their needs

Working daily within EMR to collect data, organize information, track tasks, and communicate with staff, patients, and family members; Providing feedback to the product development team, and, over time, becoming part of a super‑user group to assist in onboarding and supporting others

Requirements

You have 0-2 years of customer service or sales experience with a successful track record to share for that time period

You have a passion for working within the community you are a part of, or have been a part of in the past

You have dynamic interpersonal skills and a strong desire to improve the health of individuals and whole communities

You have a metrics‑driven approach to your work and are willing to do whatever it takes to meet quotas for the Outreach team

You are comfortable with high‑volume outbound calls as well as creative outreach

You are an independent self‑starter who responds well to change and shifting priorities

You are excited about how technology can support your work and help drive the ongoing evaluation toward new, better, care

Travel Requirements This position is remote and work would be based in your home. The necessary technology would be provided to work out of your home. Travel to headquarters in Chicago, IL for occasional meetings may be expected.

Salary And Benefits Position is full time/exempt with competitive salary and benefits package including health insurance offering. Salary range for this role is competitive depending on the candidate’s level of experience
