Job Summary The Social Worker plays a vital role in supportingand identifyingtheneed for medically related social services and pursuingthe provision of these services. The Social Worker is responsible for identifying the psycho/social and emotional needs of resident and devising and implementing services/interventions to meet those needs including collaboration with outside community resources. This position works closely with residents, families, and the interdisciplinary care team to ensure high-quality care and smooth transitions throughout a resident’s stay.

Hourly wage: $23.50– $25.00 per hour depending on experience

Key Responsibilities
Works closely withAdmissions and Care Transitions Managertodrive admissions and ensure a smooth admissions process for patients.
Interview residents and family members for possible admissionto the facility
Complete necessary forms for admission of a resident
Completenecessary forms and make arrangementsforresident'sdischarges
Attend resident care planning sessions to develop realistic approaches and goals for the resident
Documentresidents'response to the care plan in the medical record as required by state and federal guidelines.
Maintainainan up-to-date file on community services available in the area
Orient new residents to the facility
Participatein committees as assigned by the Administrator
Advise and inform Administrator of resident admissions and discharges
Act as resident advocate in the delivery of care
Communicate with and counselresidentson social and emotional needs
Visit referral possibilities and develop a working relationship with them
Screen inquires andgivestours of the facility
Provides services to meet medically related social and emotional needs of the resident
Record Information Accurately
Maintain Confidentiality of Records
Any and all other duties assigned by Administrator in an effort to maintain a quality social services department.

Qualifications
Must possessa bachelor’s degree in social work or other related field such as psychology or sociology
Must have a valid driver’s license
Must be organized and self-motivated
Must have ability to work independently
Experience in counseling is preferred
Ability to plan, organize and implement individual resident care plans
Ability to carry out public relations activities
Ability to speak to an audience
Must be able to stand or walk 4-6 hours in an8-hourday
Must be able to occasionally bend, squat, and twist bodyduringan8-hourday.
Must be able to lift a maximum weight of 20# with frequent lifting, carrying, pushingand pulling of objects up to 10#
Able to read, write and speak English.
Basic computer knowledge and ability to learn company applications.
Previouslong-termcare or skilled nursing experience
