Position Summary: As an Independent Optometrist leasing space inside our stores you will take a leading role in patient care with respect to eye and vision care as well as general health and wellbeing.

What You’ll Do

• Launch your services in Sam's Club.
• Provide patient care for eye and vision services and general health and wellbeing.
• Establish your days of practice, hours of operation, patient schedule, and fee schedule.
• Keep 100% of examination and professional fees.

• Advanced equipment/instrumentation.
• Administrative and/or Opt-Tech support services (if state allows).

Lease Agreement Qualifications

• Doctor of Optometry (OD) from an accredited optometry school.
• Valid state optometry license.
• Ophthalmologist MD or DO from an accredited school.
• Valid state ophthalmology license.
• General Liability Certificate / Professional Liability Insurance Certificate.

ME/VA license agreements are with Gumberg Asset Management Company.
