Minimum Qualifications for a Lease Agreement

• Doctor of Optometry (OD) from an accredited optometry school
• Valid state optometry license
• MD or DO from an accredited ophthalmology school
• Valid state ophthalmology license
• General liability certificate / professional liability insurance certificate
