Now Hiring: Social Services Director (SSD)
Schedule: Mon-Fri 8a-4.30p
About the Role: We’re looking for a dedicated Social Services Director to lead our facility’s Social Services Department. In this role, you’ll oversee department operations in compliance with federal, state, and local regulations and ensure that each resident’s social and emotional needs are met on an individual basis.
You’ll be instrumental in supporting residents and families through transitions, care planning, and community resources, while contributing to the overall success and occupancy of our community.
Key Responsibilities:
Lead and manage the Social Services Department, ensuring compliance with all regulations
Assess residents’ social and emotional needs and develop individualized care plans
Participate in discharge planning and care coordination
Support and counsel residents and families on services, benefits, and financial resources
Maintain accurate records, documentation, and quality assurance programs
Develop community resources and referral networks
Participate in surveys and implement corrective actions as needed
Handle inquiry calls, tours, and assist with admissions processes
Collaborate with staff on resident transitions and special needs
Serve as backup for Admissions as necessary

Requirements
Bachelor’s degree, required.
Bachelor’s degree in Social Work (BSW) strongly preferred (BSW required for communities with 120 beds or more).
3+ years of experience preferred.
