Director of Social Work

Salary: $70K Range; based on experience
Requirements
Must have prior nursing home / long-term care experience
Previous experience as a Social Work Director
Strong knowledge of discharge planning, care coordination, and resident advocacy
Experience working with interdisciplinary teams
Familiarity with state and federal regulations related to long-term care
Job Description
Plans and directs a comprehensive social services program which provides support services for residents and families.
Responsible for coordinating all admissions and discharges to ensure facility census is kept at its maximum capacity.
Participates in an interdisciplinary team approach to better serve the needs of the facility population.
Oversee and manage the Social Services department
