Magnolia Creek Nursing and Rehabilitation is seeking a Social Services Director for our skilled nursing facility. Social Services Director Job Duties
Conduct comprehensive assessments of residents' psychosocial needs and develop personalized care plans in collaboration with the interdisciplinary team.
Provide emotional support, counseling, and resources to residents and their families to address their social and emotional well-being.
Assist residents in accessing community resources and services to meet their individual needs and preferences.
Facilitate family meetings and serve as a liaison between residents, families, and the facility to ensure open communication and understanding.
Collaborate with medical professionals, including doctors, therapists, and social workers, to ensure the holistic care of each resident.
Assist residents with discharge planning and coordinate the transition to home or other care settings when necessary.
Maintain accurate and confidential resident records, ensuring compliance with all relevant regulations and policies.
Stay updated with changes in regulations and best practices related to social services to enhance the quality of care provided.
Qualifications
Minimum of a Bachelor\'s degree in Social Work or a bachelor\'s degree in a human services field, including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology.
One year of supervised social work experience in a health care setting, working directly with individuals.
Strong communication and interpersonal skills to interact with residents, families, and staff effectively.
Empathetic and caring nature, with a genuine desire to improve the lives of our residents.
Knowledge of relevant regulations and laws related to social services in a healthcare environment.
Ability to work collaboratively with a diverse team and maintain a positive working environment.
