
Overview
Develop and maintain professional relationships with referral sources including hospitals, clinics, physician offices, rehab facilities, SNF's, assisted living facilities and other community organizations. Educate referral sources on Home Health and Hospice services, eligibility criteria, and referral processes. Act as the point of contact for new and existing referral partners. Promote Home Health and Hospice programs and services while maintaining healthcare quality standards. Maximize human, financial, and equipment resources. Report directly to the Agency. Responsibilities
Provide counsel to the Agency regarding community needs, program development, personnel, and financial requirements. Collaboratively develop and maintain policies for new programs. Ensure compliance with local, state, and federal laws related to marketing the Agency. Assist in directing programs through collaboration and delegation to administrative and supervisory personnel. Partner with the Interdisciplinary Group to support safe and effective patient and family care. Establish public relations programs to interpret the Agency's services and foster good relationships with physicians and community agencies. Perform other duties as assigned by the Governing Body. Strong interpersonal and communication skills Professional presentation and relationship-building ability Knowledge of Home Health and Hospice services and regulations Organized, self-motivated, and goal oriented Strong time management and follow-up skills Ability to establish and maintain effective working relationships with staff, the public, governing bodies, and medical directors. Strong communication and collaboration skills. Education & Experience
Graduate of an accredited college/university preferred. 1-2-year experience in the healthcare field required.
