Position Summary

We are seeking a motivated and relationship-driven

Community Outreach & Referral Development Specialist

to build and strengthen partnerships with referral sources across the healthcare community. This role is responsible for establishing and maintaining professional relationships with hospitals, clinics, physician offices, SNFs, rehab centers, assisted living facilities, and community organizations. The ideal candidate is a strong communicator with a solid understanding of

Home Health and Hospice services , eligibility criteria, and referral processes. This individual will serve as the primary point of contact for new and existing referral partners, promote agency services, and ensure compliance with all healthcare marketing regulations. Key Responsibilities

Build and maintain relationships with hospitals, clinics, physician offices, rehab facilities, SNFs, ALFs, and community partners Educate referral sources on Home Health & Hospice services, eligibility requirements, and referral procedures Act as the main liaison for all inbound and outbound referral communication Promote agency programs while upholding high standards of care and compliance Program & Agency Support

Provide strategic counsel regarding community needs, program enhancements, staffing considerations, and resource requirements Collaboratively develop and implement policies for new or expanding programs Support agency initiatives through collaboration with interdisciplinary teams Compliance & Regulation

Ensure marketing practices align with state, federal, and agency regulations Partner with clinical leadership to support effective, safe, and appropriate patient care Maintain accurate documentation and comply with agency policies & procedures Public Relations & Community Engagement

Establish and maintain public relations programs to increase awareness of agency offerings Represent the agency professionally with physicians, community agencies, and healthcare partners Participate in events, community presentations, and outreach activities Support administrative and supervisory teams as needed Perform duties assigned by the Governing Body Knowledge, Skills & Abilities

Excellent interpersonal, communication, and presentation skills Strong relationship-building and professional networking ability Understanding of Home Health & Hospice programs and regulatory standards Self-motivated, organized, and goal‑oriented Strong time‑management and follow‑up skills Ability to work effectively with staff, the public, governing bodies, and medical directors Education & Experience

1–2 years of experience in healthcare

required (Home Health or Hospice preferred) Valid driver’s license and reliable transportation Why Join Us?
