Location: Remote - Work from home position in Chicagoland area in case they need to go onsite for a meeting (seldom)
The position provides support to the IL Medicaid Care Coordination team by outreaching to IL Medicaid members to verify that waiver services have started timely and to assist with trouble shooting if they have not. The position is also responsible for outreaching members to complete health risk screenings and providing coverage for any additional outreach efforts that are needed.

Bachelor Degree OR 4 years health care, social services or community advocacy experience, Knowledge of medical terminology Experience coordinating member related needs, providing assistance to members, and analyzing member needs PC proficiency including Microsoft Office applications Preferred requirements
Community Health Worker (CHW) certification from HCSC approved college Bachelor Degree OR 4 years’ health care, social services or community advocacy experience,
