We are seeking a dedicated Social Service Director to lead our team. Social Services is an integral part of our interdisciplinary team, working with therapy, nursing, and the rest of the staff to ensure the quality of the residents’ stay followed by a smooth transition back into their home life. The Social Services Director is responsible for monitoring the residents’ mental and psycho‑social needs and providing the services to meet these needs in order to attain or maintain the highest practical level of physical, mental, and psycho‑social well‑being.

Social Service Directors must have the following:
Bachelor’s degree in Social Work
2‑3 years’ experience in a social services/discharge planning role in a long‑term care or skilled nursing setting
Working knowledge of Medicare/Medicaid & insurance
As a skilled, driven Social Service Director, you will:
Develop an ongoing care plan based on the residents’ needs, interests, and abilities as identified in the initial assessment.
Participate in care plan meetings.
Complete required record‑keeping upon resident admission, discharge, transfer; chart and document per facility policy.
Implement healthcare directives and identify end‑of‑life care opportunities.
Assess the resident’s potential for discharge as a member of an interdisciplinary team.
Plan for the resident’s discharge and post‑discharge with the resident, family, and interdisciplinary team.
Assist the resident and family in the implementation of the discharge plan.
