Qualifications
Bachelor’s degree in a human services field (e.g., sociology, psychology, special education, rehabilitation counseling) plus at least one year of supervised experience in mental health or human services.
Position Summary Responsible for coordinating and monitoring psychiatric rehabilitation services for an assigned caseload in accordance with federal, state, and facility regulations. Ensures residents’ emotional and social needs are assessed, addressed, and maintained through individualized care planning.
Key Responsibilities
Complete assessments for new and readmitted residents
Develop, implement, and monitor individualized care plans and discharge plans
Provide 1:1 counseling, group services, skills training, and behavioral interventions
Facilitate care plan conferences and collaborate with the interdisciplinary team (IDT)
Maintain accurate documentation, progress notes, and required reports
Assist with discharge planning, community referrals, and family support
Coordinate services with other departments and community agencies
Participate in surveys, quality improvement, and regulatory compliance activities
Support residents in crisis situations with professionalism and compassion
Skills & Abilities
Strong interpersonal and organizational skills
Ability to work collaboratively in a fast-paced environment
Professional, patient, and compassionate approach with residents and families
Knowledge of psychiatric rehabilitation regulations and resident rights
