Purpose The Jewish Board’s Community Behavioral Health treatment programs provide compassionate, high quality, evidence‑based services to individuals and families in the communities we serve. Our staff use a culturally competent, person‑centered approach to help individuals and their families develop skills and resources to improve overall functioning, instill hope, and strengthen resiliency. Our programs work closely with community partners to address health disparities in our neighborhoods while celebrating the strengths and resilience of our communities.
Program Description The ACT program provides an integrated set of evidence‑based treatment, rehabilitation, case‑management and support services to individuals residing in shelters, living in supportive housing programs, or their own residences who are diagnosed with severe mental illness and whose needs have not been well met by more traditional service delivery approaches. Through a mobile, multi‑disciplinary team, recipients are supported in their recovery through an individualized, person‑centered model of care.
Position Overview The Team Director leads the ACT Team, ensuring clinical services are delivered using evidence‑based approaches and best practice, and manages day‑to‑day operations to ensure the program runs smoothly. The Director provides ongoing training, supervision, and guidance to clinical and office staff.
Key Essential Functions
Dedicates 50% of time to direct patient services and 50% to administrative and supervisory duties.
Manages day‑to‑day clinical and operational operations and ensures adequate coverage.
Ensures quality of services using outcomes data and client feedback.
Assists Program Director with hiring, orientation, performance evaluation, training, and addressing performance issues.
Ensures all reporting requirements are met on time, including staff documentation and system reporting.
Conducts side‑by‑side contacts with staff as part of direct patient services.
Provides regular supervision through daily team meetings, case conferences, and one‑on‑one supervision.
Leads daily team meetings and reviews meeting minutes.
Maintains a caseload equivalent to one‑half a full caseload.
Participates in intake process for new referrals and reviews ongoing service plans.
Supervises program interns or oversees their clinical supervision.
May run specialized groups or provide clinical coverage when needed.
Acts as a key resource for community outreach, establishing linkages with shelters and other organizations.
Completes all necessary notes, progress notes, risk assessments, and paperwork.
Provides after‑hours emergency coverage on a rotating basis.
Educational / Training Required A Master’s degree or higher in social work, psychology, rehabilitation counseling, or a related field plus New York State licensure as an LMSW, LCSW, LMHC, LMFT, Licensed Psychologist, or Licensed Creative Arts Therapist.
Experience Required / Language Preference
Minimum three years of related work experience.
At least one year of direct clinical supervision preferred.
Specialized experience with substance use disorders, vocational services, family therapy, and/or homelessness a plus.
Previous work on an ACT team or Mobile Crisis team a plus.
Computer Skills Required
Proficiency in Microsoft Office.
Proficiency with electronic records and data entry for medical notes.
Visual and Manual Dexterity The candidate should be able to read paper and electronic documents and perform significant data entry into various computer programs.
Work Environment / Physical Effort 50% of the work is performed in the community and 50% on site.
