Summary The Document Service Representative assumes full responsibility for processing incoming mail for the organization and handling all incoming paper claims and correspondence documents. This role involves tasks such as detailed mail sorting, batch preparation, and scanning to ensure compliance with New York State regulations and accurate image routing to support customer service levels. The representative is responsible for claims examining and data entry on the Form Works system, ensuring required data elements are captured and images are filed appropriately. Collaboration with various departments is necessary to resolve claims and correspondence issues, applying problem‑solving and time‑management skills to balance multiple tasks while maintaining production and quality standards.

Responsibilities

Examine, data enter, and re-write all physician, hospital, dental, and pharmacy claims using established policies and procedures.

Perform accurate FormWorks OCR error correction, Key from Image, and Key Entry on claims in accordance with departmental desk levels.

Report scanning, data entry, or workflow issues to the Team Lead or management as appropriate.

Work mandatory overtime as necessary to meet TAT service level metrics.

Provide feedback on established desk levels and train new staff when required.

Maintain production and accuracy ratios as established by the department.

Research and contact resources for information necessary for claims examining and data entry.

Initiate and assist in process improvements to resolve claims examining and data entry issues.

Perform all scanning room functions, including sorting, prepping, and scanning documents into FormWorks.

Archive incoming claim and correspondence documents in accordance with established Desk Levels.

Verify scanned documents for completeness and report any system or workflow issues.

Perform general scanner maintenance as needed.

Comply with the company’s Corporate Compliance Policy and report any suspected violations.

Qualifications

High school diploma or GED required.

Six months of alphanumeric data entry experience in a healthcare setting is required.

Minimum of one year of claims examining procedural review experience utilizing CPT-4, HCPCS, and ICD-9 codes is highly preferred.

Experience with online data entry systems, especially Macess EXP and FormWorks, is highly preferred.

Strong PC skills and experience with Microsoft Windows are preferred.

Demonstrated ability to solve problems related to claims examining and data entry.

Ability to apply problem‑solving and time‑management methodology to balance multiple tasks.

Strong attention to detail and ability to meet quality and production standards.

Ability to work independently with minimal supervision.

Pay Transparency The typical base pay for this role across the U.S. is

$20.00 - $21.00/hr.

Non‑exempt positions are eligible for overtime at a rate of 1.5 times the base hourly rate for all hours worked in excess of 40 in a work week, or as required by state or local law. Final offer amounts, within the base pay set forth above, are determined by factors including your relevant skills, education and experience. Full‑time employees are eligible to select from different benefits packages.
