Community Outreach Specialist
to join our interdisciplinary PACE team in
Tampa, FL . In this role, the PACE Community Outreach Specialist is responsible for driving program growth through strategic community engagement, relationship development, and high-quality referral generation.

The Program of All-Inclusive Care for the Elderly (PACE) is a unique care model designed to help older adults remain safe and independent in their own homes.
Relationship-Based Care with Real Impact: Build long-term connections with participants, supporting them through all stages of care—this is a meaningful alternative to traditional inpatient or facility-based care.
Team-Driven, Preventive Focus: Collaborate closely with physicians, social workers, therapists, and other clinicians in a proactive care model focused on prevention, wellness, and holistic well-being.
What You’ll Do
Build and maintain strong relationships with community partners, referral sources, senior-serving organizations, and healthcare providers.
Develop and execute outreach strategies to increase awareness of the PACE program.
Generate high-quality referrals and guide prospective participants and families through the enrollment process.
Represent the PACE program with professionalism, empathy, and a deep understanding of geriatric care needs.
Meet and exceed performance metrics tied to outreach activity, referral volume, and enrollment growth.
Identifies populations of Medicaid, Medicare, Dual Eligible, VA, and Private Pay individuals and strategically aligns Empath Life PACE services within their communities through referral channels.
Serve as a brand ambassador in the community, strengthening program visibility and reputation.
Maintain a thorough knowledge of the community’s health care market, competition, consumer demands and governmental policies which influence the agency’s outreach goals and access for patients/families.
Creates and conducts effective proposal presentations to referral sources and prospective participants.

What You’ll Need
Bachelor’s Degree in marketing or healthcare administration preferred
Minimum of two (2) years of experience in health plans or senior-focused programs, with demonstrated success meeting or exceeding sales goals
At least two (2) years in a healthcare or senior care setting preferred (experience within the service area and outside sales experience strongly preferred)
Strong customer service orientation
Ability to work independently with minimal supervision
Experience with CRM software preferred
Bilingual skills a plus
