Community Life Specialist
Position Type : Full-Time, Non-Exempt

Our 28-acre campus includes a Skilled Nursing Facility, Long-Term Care Nursing Services, Memory Care, Assisted Living and Independent Living Homes. Shift:
Monday-Friday 8:30am-5:00pm
Position Summary:
Supports and promotes Resident well-being by assisting with resident engagement and life enrichment needs. Provides opportunities for residents to access both on and off the community campus via transportation services.
Qualifications
High School Graduate or general education degree (GED).
5+ years Driving Experience
1+ years related social or recreational program experience.
Current Washington Driver’s License, Auto Insurance and Safe Driving Record
Essential Duties and Responsibilities
Creates and maintains an environment that translates the mission of eliseo into actions that are conducive to the maximum well-being of the residents served.
Assures that department services are of appropriate quality and are consistent with eliseo philosophical principles.
Facilitates coordination among all departments & divisions of eliseo in delivery of services.
Ensures consistent action is taken by all employees, volunteers, and visitors to comply with HIPAA security/privacy regulations regarding confidential patient records and information.
Maintains in-house channel and resident engagements platforms.
Assists with maintenance of eliseo mass notification and resident emergency alert systems.
Provides transportation for residents on and off campus, including driving, scheduling, escorts residents to assure safety between vehicles and pick-up/drop-off locations.
Ensures delivery of printed and digital information and occasional meals, etc to residents.
Checks vehicle, weather, and driving conditions to determine viability of safe operations.
Logs vehicle settings in logbook to include mileage, fuel level, and reports any issues with vehicle.
Transports residents using multiple passenger vehicles, with or without lift capabilities, including wheelchair accessibility.
Assists residents with loading/unloading of resident equipment to include wheelchairs, walkers, canes and personal items.
Reports any changes in resident status, unusual occurrences or behaviors and any problems with completion of appointments.
Supports with various administrative tasks as directed by Resident Services Director.
Represents eliseo in a professional manner by demonstrating positive public relations.
