Position Summary Prestige Care Center of Fairfield is seeking a dynamic and experienced Director of Rehabilitation (DOR) to lead and coordinate our therapy services department. The Director of Rehabilitation is responsible for the overall clinical quality, operational management, and regulatory compliance of the rehabilitation program within our skilled nursing facility. This role ensures the delivery of high-quality therapy services while supporting interdisciplinary collaboration and positive patient outcomes.
The ideal candidate will be a licensed Physical Therapist (PT), Physical Therapist Assistant (PTA), Occupational Therapist (OT), Certified Occupational Therapy Assistant (COTA), or Speech-Language Pathologist (SLP) with strong leadership, communication, and organizational skills.
Key Responsibilities
Provide leadership, supervision, and coordination of the rehabilitation department including PT, OT, and Speech therapy services.
Ensure delivery of high-quality, evidence-based therapy services that meet resident goals and regulatory requirements.
Oversee therapy scheduling, staffing, and productivity to meet operational targets while maintaining quality care.
Collaborate with physicians, nursing staff, case management, and interdisciplinary teams to ensure coordinated resident care.
Monitor therapy documentation for accuracy, timeliness, and compliance with federal, state, and payer guidelines.
Maintain compliance with Medicare, Medicaid, and other regulatory standards.
Participate in care plan meetings and ensure therapy goals align with resident-centered outcomes.
Monitor clinical outcomes and implement quality improvement initiatives within the therapy department.
Provide mentoring, coaching, and performance management for therapy staff.
Assist with program development to enhance rehabilitation services and resident outcomes.
Participate in facility leadership meetings and contribute to strategic planning initiatives.
Qualifications
Must hold a current license in the state of Iowa as one of the following:
Physical Therapist (PT)
Physical Therapist Assistant (PTA)
Occupational Therapist (OT)
Certified Occupational Therapy Assistant (COTA)
Speech-Language Pathologist (SLP)
Minimum
2–3 years of rehabilitation experience
in a skilled nursing facility, rehabilitation center, or similar healthcare setting preferred.
Leadership or supervisory experience
in rehabilitation services preferred.
Strong knowledge of Medicare Part A, Part B, PDPM, and therapy documentation requirements.
Excellent leadership, organizational, and communication skills.
Ability to manage multiple priorities and maintain regulatory compliance.
