We are looking for an Admissions and Activities Social Services Director!
An Activities/Social Service Director cares deeply about the psychological, social, and spiritual needs of residents. The ideal candidate for this job will appeal to residents from all backgrounds, providing an opportunity to celebrate individuality through expressive and creative activities. Responsibilities and Purpose
Initiate and promote activities both within and outside the institution to stimulate and promote resident interest and general well being.
Encourage social communication among residents through group activities, recreation and one-on-one activities.
Encourage intellectual and educational development through literature, lectures, movies, cultural events and other activities available in the community.
Primary contact for all telephone admission inquiries, walk-in admission inquiries and request for pre-admission facility tours.
Directly responsible and accountable for the completion of all admission processes, forms and facility notification documentation.
Coordination and regular communication with the Central Intake Team and other referral sources, ensuring a smooth patient discharge and admission experience.
Identify each resident’s social, emotional and psychological needs.
Develop and carry out a plan to develop the residents’ full potential during their stay.
Planning for the resident’s discharge or transition to another care environment.
Comply with safety regulations and policies. Knowledge of federal and state guidelines.
Required Experience and Education
Bachelor’s degree in Social Work (BSW) or Bachelor’s degree in human services field (including, but not limited to, sociology, special education, rehabilitation counseling, gerontology and psychology).
1+ year of supervised social work experience in a healthcare setting working directly with individuals.
Completion Certificate for Oklahoma-approved Social Services training program.
Basic computer skills, including Microsoft Office.
Knowledge of senior healthcare services, specifically Skilled Nursing and Long Term Care.
Knowledge of Medicaid, Medicare, and third party managed care payer sources.
Business Development/Networking experience a plus.
Experienced in customer service and developing relationships within the senior healthcare environment.
