Regional Director of Care Management and Community Partnerships

Job Purpose or Summary: In alignment with the company’s mission, vision, and values, the Director of Community Partnerships is a senior market-level leader responsible for building deep, trusted relationships across the local care ecosystem to drive sustainable growth and strengthen Arosa’s brand presence. Serving as a peer to the Regional Director, this role focuses primarily on strategic partnership development, brand awareness, and community education, ensuring that referral partners, families, and professionals clearly understand Arosa’s care management and home care services. In addition, this leader supports the development of care managers and field leaders by modeling how to naturally integrate community education into everyday practice, and maintains limited direct involvement in billable care management to stay grounded in the client experience.

Essential Duties & Responsibilities: Community Partnership & Brand Leadership (Primary Focus – ~80%)

Establish and maintain senior-level relationships with key referral partners including hospitals, physician groups, senior living communities, elder law and fiduciary partners, care managers, and community organizations.

Serve as a visible ambassador of the Arosa brand, representing the organization at industry events, professional associations, and community forums.

Develop and execute market-specific partnership strategies to expand awareness of care management and home care services.

Identify new partnership opportunities that align with market growth goals and long-term brand positioning.

Build trust-based relationships that position Arosa as the partner of choice for complex care needs.

Market Development & Growth Strategy

Partner closely with the Regional Director to align community strategy with operational and growth priorities.

Provide insight into referral patterns, market dynamics, and emerging community needs to inform regional strategy.

Support the launch of new initiatives, services, or care models through targeted community education and partner engagement.

Track and evaluate partnership impact using defined success measures (e.g., referral volume, relationship depth, referral conversion).

Team Development & Education

Serve as a mentor and resource to care managers and local leaders on how to incorporate community education into daily interactions.

Support training efforts that strengthen comfort and skill in relationship-based growth, storytelling, and professional presence.

Model best practices in professional outreach, follow-up, and partnership stewardship.

Limited Direct Care Involvement

Maintain a small, intentional portfolio of billable care management work to remain connected to client and family needs and ensure community messaging reflects real-world experience.

Supervisory Responsibilities

This role may provide functional leadership and influence across care managers and community liaisons but does not carry formal people-management responsibility unless assigned by leadership.

Knowledge and Skill Requirements:

7+ years of experience in healthcare, senior services, care management, or community-based leadership roles.

Demonstrated success building senior-level partnerships in complex care environments.

Strong executive presence with the ability to engage credibly with physicians, attorneys, administrators, and community leaders.

Proven ability to influence without authority and work cross-functionally with operations and clinical teams.

Excellent communication, presentation, and relationship-building skills.

Valid driver’s license and proof of current auto insurance required.

Physical Demands and Work Environment:
