ABOUT THE ROLE The
Community Outreach Leader
is the face of Hopscotch in the local community and plays a key role in connecting eligible patients to care. This person builds trust, strengthens partnerships, and drives patient engagement through consistent outreach and relationship-building. You’ll help ensure community members understand Hopscotch’s services and can access care—especially in rural and underserved areas.
This is both a relationship-based and results-driven role. Outreach activity is tracked, measured, and reported, and you’ll be expected to work toward KPIs and enrollment targets while maintaining an empathetic, high-integrity approach.
WHAT YOU’LL DO
Build and maintain strong relationships with community partners (churches, schools, senior centers, housing programs, food pantries, clinics, etc.)
Represent Hopscotch at local events, community meetings, health fairs, and outreach opportunities
Educate community members about Hopscotch’s model and services in a clear, compassionate, and approachable way
Confidently represent Hopscotch in the community by presenting to small and large groups (e.g., community events, partner organizations, senior centers)
Support patient growth by identifying potential patients, encouraging engagement, and helping remove barriers to care
Maintain a pipeline of community leads/prospective patients and follow up consistently
Track and document outreach activity, leads, referrals, and enrollments using internal tools and processes
Collaborate closely with clinic and operational teams to align outreach strategy with patient growth priorities
Use outreach data and local insights to improve outreach efforts over time
Provide basic patient navigation support as needed (scheduling support, transportation resources, community resources)
ABOUT YOU You would be a great fit for this position if you are interested in building connections with our community, serving prospective patients and educating them about us, and strategically working to expand our patient base to best serve the community and Hopscotch Primary Care.
You have experience in community outreach, sales, public health, social work, patient engagement, enrollment, or related work.
You’re warm, engaging, and naturally build trust with diverse populations.
You’re comfortable working independently in the field and taking ownership of your day-to-day priorities.
You’re highly organized and consistent with follow-up and documentation.
You’re comfortable being held accountable to and have experience meeting sales goals, KPIs, and performance targets — and you like tracking progress and outcomes.
You communicate clearly and confidently (in-person, phone, and text).
You’re mission-driven and passionate about improving access to healthcare.
You have reliable transportation and can travel locally in your territory.
The following are preferred but not required:
Degree from a 2- or 4-year institution
Two to three years of territory sales management experience
Two to three years of healthcare sales experience.
Driving and Motor Vehicle Compliance Driving is an essential function of this role. Employees in this position are required to operate a personal or company vehicle for business purposes. As a condition of employment, candidates must:
Possess and maintain a valid, unrestricted driver’s license
Provide and maintain current documentation of vehicle registration and proof of insurance
Maintain a driving record that meets company standards
Consent to and successfully pass a Motor Vehicle Record (MVR) check prior to employment and at intervals determined by the company
