Job Description We are seeking a dedicated and experienced Registered Nurse (RN) to join our team. The primary responsibilities of this role include conducting client visits, assessing client conditions and environments, developing Activities of Daily Living (ADLs) plans based on client needs, and evaluating caregiver performance.
Key Responsibilities
Conduct comprehensive client visits to assess health conditions and living environments.
Develop individualized care plans, including ADLs, based on specific needs.
Monitor and document client progress and adjust care plans as needed.
Provide education and support to clients and their families regarding health management and ADLs.
Evaluate caregivers’ performance to ensure quality care per established plans.
Collaborate with healthcare professionals and caregivers for holistic client care.
Maintain accurate and timely medical records and documentation.
Ensure compliance with all healthcare regulations and standards.
Participate in ongoing training and professional development.
Qualifications
Valid and current Registered Nurse (RN) license in South Carolina.
Minimum 1‑2 years of nursing experience, preferably in home health care.
Strong clinical skills and experience in health assessments.
Excellent communication and interpersonal skills.
Ability to develop and implement ADL plans tailored to individual client needs.
Experience evaluating and providing feedback to caregivers.
Ability to work independently and manage time effectively.
Compassionate and patient‑centered approach to care.
Valid driver’s license and reliable transportation.
